A plan to support a young person’s transition from high school into adult life. These might be called something different, depending on your school and which State or Territory you live.
An individual transition plan (ITP) may be part of a separate plan, or part of your child’s individual education plan.
Families should be involved in developing what’s in their young person’s ITP, and in how it’s used to support their transition into adult life.
